It's getting to be that time of the year again

So as winter fast approaches I am reminded of a constant battle that I have with the cold and I was looking for suggestions!
When working outdoors how long should you equalize the temperature of the camera and lens to stop fogging? ( I try to leave my gear in a secure area at outside temp less battery)
What's the best way to maintain battery life in the cold? ( I normally have a spare under my shirt)
How can you overcome temperature differential causing shimmer or scintillation. ( this always happens when the light is perfect but I have to use a long lens!!)
