Focusing problem with D800 and 70-200 VR

I have a 70-200mm f2.8 VR lens which is my workhorse lens (not the VR11)
I seem to have focus issues when I have it on my D800.
I would frame a persons, move the focus point to her eye or nose but every 3rd or 5th shot the lens would focus on something behind her even though the focus point is on her face.

I dont know if I am doing something wrong, if its my lens or the camera ??
