About to purchase a D7000

Hi guys, I'm new here. I'm a dentist based in the UK and am keen to get in photography both clinically and recreationally.

I'm pretty much decided on the D7000 but am unsure whether to buy with the kits lense or not. I'm going to get a dedicated macro lens and flash for the clinical work but am unsure what to go for the recreational side. I have no previous experience with slr's.

I've read good things about the 18-200 nikon and also the latest 18-270 tamron.

Is the supplied kit lens 18-105 any good?

Also any recommendations on a macro lens? It's coming down to a choice between the 90mm tamron, 105mm sigma or Nikon.
