So, the plan is a month of romping around PI. Leaving in January. Now, I haven't been there in 21 years!! I've been hearing from family members that people are dropping like flies with mosquito related illnesses--namely the West Nile virus and/or Malaria.
1. Do I need to get extra vaccination shots? If so, which ones? (My main destinations are Ilocos and other provinces with stops to Manila/Makati).
2. Know of any connections to "cheap" flights roundtrip? Doesn't have to be PAL but I've been told it's easier to go through customs if flying via PAL.

My last two trips back were in 2004 and 1994 and I didn't take any extra shots. But then, I grew up in Manila but my family has been in the US for 18 years so you can say that my immune system is geared for the Philippines. If you are worried abou this, best to ask your favorite M.D.
I've sourced flights one of two ways. Tapping into the local Filipino community, there's always someone who runs a travel business and gets access to good deals. But in the internet age, you should also check Travelocity etc to make sure there aren't better deals there. Coming from Chicago, I've taken AA/JAL and United the last couple of times I've gone back. I can't get to Manila non-stop so I tend to get the option with the least travel time regardless of price.
I've never flown PAL into Manila so I can't comment on relative ease of going through customs when flying with PAL. I had no issues with customs my last two trips back. Going through customs used to be really, really bad but when someone asks for "pasalubong" - their euphemism for grease money, I just tell them to go ahead and inspect my bags. They wave me through everytime.

anyway, unless you're going to the remotest regions of the country, there is really no need for vaccination.
i've never heard of any west nile cases here. malaria is prevalent in some remote areas, e.g., st. paul underground river in palawan, but OFF lotion can take care of that according to our tour guide last year.
we have no bird flu and no SARS here

No need of vaccine but be sure you have to check that you've received the following vaccine while you are a child and get boosters if needed.
1. Hepatitis A and B
3. TB vaccine
4. Chiken pox
Pag meron ka na non OK na. Malaria is not present in your destinations.
I've stayed in Puerto Priincesa for 2 years and been to underground river a couple of times. I did not use off lotion or took some malaria prophylaxis...
Don't drink the water from the faucet yong bottled water..baka magkaroon ka ng traveler's diarrhea similar to the one if you go to Mexico.
