Settings for flash on D100

I have had a lot of trouble with an SB-28dx on my D100. I set the camera in manual to 1/200 (defaults to 1/180), f5, ISO 200 and the flash to auto mode. My relatively close photos (less than 10 ft)are all underexposed by 1 to 2 EV.
I am curious to hear how you are setting your flash, both external and built-in, and what kind of results you are getting.
