Here in Switzerland, you save CHF 150 if you buy a kit of D200 plus 18-70: 2598 + 648 = 3246 - 150 = 3096, it means EUR 100 in EU and very likely USD 100 in the USA. Unfortunately, no discount if you want D200 + 18-200 (1148)...
The 12MP D2X is known to have such high resolution that it shows the flaws of all but the best optics. A 10MP D200 will be close to that. Therefore, a superzoom lens like a 18-200 may be only a match of convenience but not yield the quality images one invests in a 10mp camera since a superzoom is bound to have distortions here and there. Therefore, I doubt this will or should be a kit lens for a D200 or D2X. But it may be great for a D50/70.
Of course if the 18-200 turns out to be sharper and better optical quality than the 24-120VR, we'll all eventually buy one.

I think they are avoiding the 'appearance' of a high price. If you or dealers want to add the lens in a package that is fine with them, but I don't think they want to officially sanction such a high-price that the D200 + 17-55 would require.
