I have only used bracketing a few times, so I may be doing something wrong. When I set for 5 bracketed shots, it only produces 4. Any bracket setting produces one less image and shutter click.

All I can tell you is my D4 does not have this problem. Ask for 5 brackets, press the shutter 5 times and it produces the expected 5 images with the expected differential in exposure (assuming I am bracketing exposure). On Cl or Ch, hold the shutter (or more likely cable release) until the shutter goes off 5 times.
Sounds like a camera problem, but you might try a reset or firmware update. Is your firmware: A:1.05 / B: 1.03?

My D4 works fine with bracketing. If you release the shutter before the 5 exposures have been completed, you will get less exposures and you will have one more to be completed the next time you trigger the exposure.
It is easy to not complete the 5 exposures if the last two have a very slow shutter speed.
