Yes, my local camera store swapped my faulty D4 for one fresh off the boat and it did not include the reader or card. The old camera had a sticker on the outside of the box letting one know that it's included. The newer camera of course did not have this sticker. They did let me keep the one from the original, although I really don't have a use for a 16g card when 32g are available. The reader being USB3 has no use to me either because my MBP does not yet support it without adding a bunch of hardware. Supposedly the new MBP will have the uSB3 ports. I wonder if they are ditching the "thunderbolt".
If Nikon is not including the card and reader, they should drop the price accordingly, or make some other equitable accommodation for the purchaser of the camera! They certainly were not giving them away.
I picked up my D4 from Roberts on Friday May 18. The XQD card and reader were included in the package.

The D4 with the XQD card and reader deal is completed. If you are getting it now, it is because it was shipped to the dealer last month. The reason it was added initially is because Sony was not going to widely available at the release. In fact, it appeard it would not be available for purchase in time. So, Nikon included it. It was stated in the beginning according to Nikon Rumors reporting of the included card and reader, that it was only going to be given till May. Hardly stopped "without warning". I was fortunate and had an early D4 and got the freebie. An extra card showed up within days of my body being delivered, from B&H. I thought is was nice to get the deal, but was unexpected. I've never had a memory card included with a body in the past.
I need to restate my position on the XQD card as a backup. If you read the reviews on B&H the reviewers rave about the card. One said that he wished the camera came with two XQD slots instead of one CF and one XQD!
