I don't believe this was a setting issue. I tried all the live view lens servo modes and none would focus in live view. Certainly I don't see why any user setting (other than manual mode) should prevent it from focusing at all in live view mode. It seemed to be similar to the problem referenced at http://www.flickr.com/groups/d600club/discuss/72157631632311164/.
As to the latter issue I am not talking about continuous AF mode but continuous shutter release mode (set using the lower release-mode knob beneath the exposure mode knob). The camera would not shoot continuously in either the continuous high or continuous low modes.
