This would seem to be a case of missed focus due to the combination of poor lighting and a low-contrast target.Aiming at the scarf's knot where the two yellow lines meet might have worked better. Also aiming for the face/teeth.If you look at the EXIF data in the original, unedited photo, where does it say the focus point was?
