A Sidekick on a monopod is very useful. You do not need a ballhead to make it work. Manfrotto makes a tilt head that mounts on the monopod and you then attach a Arca-Swiss type clamp to the tilt head and insert the Sidekick. The Sidekick handles the gimbal motion and you rotate the monopod for panning. RRS has a blurb on this set up. I have used it for years with lenses up to 500mm f4 and it is great.
