I think somewhere in Nikon's literature they recommend using spot metering when using flash and manual focus lenses. I use manual focus lenses on all my Nikon bodies and find that exposure is under exposed when metering mode is other than spot when using flash. The amount of under exposure is not consistent and somewhat depends on room darkness and distance from flash to target. It also appears worse with wide angle lenses.
Here is a very quick series of three shots I just took using the D200, a SB-800 and an AI 50mm f1.4 lens. The difference is a little more than 1 EV. I have seen differences of over 2 EV.
From left to right. Spot , Matrix , Center weighted.
edit: I just ran the same test with my new D800 using the same lens and flash. I was surprised to see the exposures were nearly identical and within .05 EV. An undocumented improvement? Attachment#1 (jpg file)
