"D300 and birds"

I am using my camera to photograph birds mainly. I am mostly using a 300mm/f2.8 with a teleconverter. I am having all sorts of problems with focus. I have yet to achieve any really clear shots. I wonder if anyone could give me any advice as to how I should have the camera set up. Most of the birds are small and in trees or bushes and the light is very inconsistent. thanks for any help.
