That goop on page 99 is probably a bad translation from the Japanese unless things have radically changed with the D4. To remove focus/refocus abilities from the shutter button, you do need to have Custom Setting A4 set to "af-on only". With it set to "af-on only", when you release the af-on button the camera will not focus again until you press the af-on button; the use of the work "lock" would seem to be confusing here.

If setting A2 is set to "release", the shutter will always be released when you press the shutter button thus possibly resulting in out of focus pictures. If set to "focus", the shutter should be held until the selected focus point becomes in focus. Are you seeing different behavior?
