"Inserting Card into Reader does not cause LR3 Import screen to appear"

I have set the preferences in LR3 under the General tab to "Cause the Import screen to appear upon insertion of a Memory Card into the Reader", but it doesn't automatically appear. I can summon it by pressing Ctrl-Shift-I.

Any ideas as to why it isn't working the way its supposed to?

One possibility is that Windows does not auto-detect new memory cards. If you have Windows set to take no action when a memory card is inserted, Windows may not report the memory card to Lightroom. Another point to check (I’m not at my Photoshop computer now): are there separate entries in LR settings for detection of new memory cards and detection of new cameras? Older versions of Windows treated memory cards like disk drives, newer ones see them as cameras.

I've had similar problems, but intermittently, with LR4. I'm not sure what causes it to appear or disappear, either. It happens on two different systems, too - just very occasionally. Like once every other month, per system, or something vaguely like that. And I'm on Macs, too.
