"XMP, IPTC, EXIF - I'm Confused"

What is the difference between these three? According to Wikipedia, all are formats for adding metadata to various file formats.

Yet I see posts that seem to speak of EXIF as the place where IPTC and sometimes XMP data are stored - "the EXIF header", "XMP/IPTC data stored in the EXIF header", etc.

Also, NX2 says you can store both XMP and IPTC data in the EXIF header. How do you choose/tell whether the metadata you type in is being stored in XMP or IPTC format? Or does it matter?
