If I need to tilt this non-tilt softbox, what bracket will I need? Also, I have read in reviews that the Speedlight bracket on this softbox does not hold the Speedlight securely. Any suggestion on a secure bracket and cold-shoe?

To ensure that you can tilt the whole contraption, you need to mount the softbox & bracket (the combo referred to in #2 in your list, above) on an umbrella adaptor on your light stand. There are lots of them to choose from - I'm a sucker for Manfrotto products. Here's what I use:

you can save some money on the hotshoe adaptor cord. Since the Lastolite will be close to the camera, instead of buying the genuine Nikon SC-28/29 cord for $60+, BH sells the Vello brand imitation cords at about 1/4 the price.
I use it for a Paparazzi-style Custom Brackets rig. At about 18", it avoids dangling, twisting, snagging.
